CHECKLIST OF APPLICATION FOR RETENTION

1.      Application for retention duly signed by the applicant;

2.     Investigation Report on Retention duly accomplished by the MARO and the ARPT assigned in the area;

3.      Sworn statement from the applicant attesting her total aggregate landholdings within the Philippines;

4.      Certified true copy of Certificate of Title;

5.      Original copy of the Municipal Assessor’s Certification of the applicant’s total aggregate landholdings, and that of the applicant for Award;

6.      Original copy of the provincial Assessor’s Certification of the applicant’s total aggregate landholdings, and that of the applicant award;

7.      Sketch Plan of the applied for retention prepared by a Geodetic Engineer duly signed by the applicant;

8.      Birth Certificate of the applicant for award and that of his children.

9. Sworn Statement of the applicant for award; and

10. MARO’s categorical Certification in relation to P.D. 27
